If you can sanitise that log of any private information and copy/paste some of it I might be able to give you some more help.
#Ring group freepbx series
Follow Me, Ring Groups, etc) Extension/User CIDs if checked. This week’s FreePBX ® Technical Series will discuss the different Ring Group and Follow-Me ring strategies in FreePBX ® 2.2 and what is coming next. You can also run asterisk -r at the SSH shell to get an interactive Asterisk shell, type core set verbose 3 to enable debugging and make a test call, you should see the server running through the dialplan and ringing extensions when the call comes in, which might shed some light on it. FreePBX is a web-based open source GUI (graphical user interface) that controls and manages. If so, I'd try upgrading the ring groups module (Admin>Module Admin) and reload again. When a call comes in to the General number, 412 + 413 rings. Both Ring Groups are set to the 'Simultaneous' Strategy and all individual destinations have a timeout of 30 seconds. The fallback (used after 30 seconds) also contains these numbers but another 5 extra. If the extension you're trying to add isn't in there then something is potentially going wrong with config generation. The Ring Group General contains two numbers, 412 and 413. You should see a line with DIAL_OPTIONS and a list of extensions, hyphen-separated. If you've got SSH access to the server, you can check the generated configuration something like this grep 1234 /etc/asterisk/extensions_nf Similarly, check there's no unconditional forwarding set in the extension properties in the web UI. Dial *73 on the phone to try clearing it if you're not sure. Did you re-use an existing extension? I've seen leftover server-side call forwarding break things before.
